Monday through Friday, 32-40 hour position responsible for case management of patients referred to the Community Health Team (CHT).
This includes appointment tracking and follow up; direct patient care; monitoring appointment attendance; data collection and process improvement; partnering with providers, hospital and the community based organizations to coordinate patient access to health care and social services.
Job Requirements:
Minimum of 2 years clinical RN experience; experience coordinating community services; knowledge of existing community-based health care; ability to successfully work with Blue Print providers, CHT, and other health care professionals; and computer competency.
