Hospice On-Call RN Full Time Contract!

Maxim Staffing is looking for an experienced Hospice RN for a contract opportunity with one of our Hospice clients in the Northwest Suburbs. This position would start ASAP and will be for 13

Work Hours: 8:30pm - 9:00am OR 5:00pm - 8:00am, including weekends. 40
hours per week.
KEY JOB BEHAVIORS
o Expectation is to complete 2 admissions per day demonstrating high flexibility, organization skills and Point of Care documentation.
o Maintain conversion rate based upon organizational objectives.
o Demonstrate effective communication following visits to facilitate identified changes to Plan of Care.
o Documentation targeted to reflect diagnosis for admission, Plan of Care, coordination of care and next steps to focus on with next visit.
o Participate in one Educational Presentation or committee involvement every year.
o On Call visits as directed by triage staff with a response to triage within 15 minutes of being called/texted.
o Attend death visits, change of location, level of care and post fall visits unless documented refusal.
o Documentation, communication and coordination to reflect Policy and Procedure.
o Make appropriate referrals as indicated.
o Must be able to take a referral.
o Submit timesheets on the dates noted on the payroll calendar unless director has given alternative approval submission date.
o Clinical staff to call at the beginning of their scheduled shift to check in and verify schedule.
o Have working knowledge of all services offered by the CareCenter.
o All nurses cross trained to be proficient in performing patient visits and death calls.
EXPERIENCE
Ability to assess and apply critical thinking skills in areas such as physical assessment, disease physiology and disease progression, symptom management and pharmacology.
Knowledge of the CareCenter programs and eligibility/administrative requirements.
Knowledgeable of Medicare and Medicaid standards and eligibility for the CareCenter programs
Knowledge of interpersonal and group dynamics
Flexibility and ability to adapt to unexpected changes in a fast-paced environment
Computer literacy; specifically, MS Outlook, Allscripts.
General nursing experience.
Knowledge of drugs and drug interactions.
Ability to quickly establish rapport with patients, family and facility staff.
Ability to transfer knowledge to skills required for patients to access appropriate care.
Excellent verbal and written communication skills.
Comfort with end of life issues and care.
The Registered Nurse, as a professional practitioner employed by Maxim Healthcare Services, Inc., assumes responsibility and accountability for the application of the nursing process and the delivery of patient care. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision. The nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
Essential Duties and
Responsibilities:
o Utilizes the nursing process to assess, plan, implement and evaluate patient care.
o Performs focused interview to identify specific patients needs.
o Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
o Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
o Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
o Identifies and prioritizes actions based upon patient care requirements.
o Monitors consistency between patient plan of care and actions of medical staff.
o Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
o Provides a safe, comfortable and therapeutic patient environment.
o Initiates and evaluates patient and/or family education.
o Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
o Documents patient's response to nursing and other interventions.
o Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.
o Monitors the safety and effectiveness of the environment and equipment.
o Participates in activities to facilitate good interpersonal communications with the chosen facility.
o Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4)JCAHO, state and local laws; 5) Patient's Bill of Rights; and 6) Maxim Healthcare, nursing and client hospital policies.
o Other duties as assigned
Minimum Requirements:
o Current Registered Nurse License for the state in which the nurse practices.
o Full command, verbal and written, of the English language.
o Current Health Certificate (per facility contract or state regulation).
o Current PPD or Chest X-Ray.
o One year prior professional nursing experience preferred.
o Current BLS card.
o Successful completion of screening tests for the clinical specialty area(s) in which the RN practices with a score of 80% or greater
o Maxim background screening

o May work at off-site facility/ client site
o May be exposed to blood-borne pathogens, chemotherapeutic and toxic chemicals

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
