LVN/RN Patient Care Coordinator
Established Home Health agency in Cedar Park is accepting applications for a full-time LVN/RN Patient Coordinator. The PCC will be responsible for coordinating patient care between field staff, doctors and agency. Must also be able to read and follow-up with lab results to provide accurate information to provider. Needs to be able to handle patient concerns and/or complaints in a timely manner. Qualifications:
Licensure for LVN/RN and 2 years experience in home health preferred.
Computer literate, Kinnser experience a plus
Exceptional organizational skills and ability to multitask
Able to execute tasks in a timely manner.
Strong customer service background and excellent communication skills.
Other Duties as required and assigned.
Estimated Salary: $20 to $28 per hour based on qualifications.
