Registered Nurse - Case Management

Registered Nurse - Case Management -- Full time - 13 week contract with potential ongoing -- Day Shift Schedule -- 8 Hour Shifts - job duties include:
1. Evaluating patients, families and home environment for admission to and continued service from the agency as per policy.
2. Coordinates POC, identifies patient problems and nursing needs, develops measurable realistic goals, specific nursing interventions including frequency of visits, and revises POC based on changes in patient status.
3. Completes comprehensive assessment (including OASIS) accurately and completely at the specified time points, within required time frame.
4. Acts as a liaison between the patient, the health team, physician and administrative staff.
5. Requests additional or diminishing service as determined by patient's needs.
6. Attends care conference as per department policy. Completes care conference notes on multidisciplinary patients.
7. Prepares, delivers and explains Home Health Advance Beneficiary Notice (HHABN) to patient/caregiver within the appropriate time frames, using the appropriate for option.
8. Prepares, delivers and explains the Notice of Medicare Provider Non-Coverage form to patient within the required time frame.
9. Develops and implements discharge plans, completes discharge criteria checklist, discharge summary and discharge instruction sheet.

Active Washington State License
Current CPR/BLS

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
