RN Case Manager
The RCM role requires an RN with experience in the care of the geriatric residents as evidenced by completed courses and clinical practice as well as current knowledge of MDS.
A current, unencumbered license to practice in the State of Oregon as well as 1 year minimum in a supervisory/management/leadership role is also required.
The RCM is responsible and accountable for managing the over-all nursing care of their assigned residents.
This will include supervising and coordinating with all members of the health care team to develop and maintain a high quality of nursing care for residents, ensuring the physical, mental and psychosocial well-being of each patient.
Estimated Salary: $20 to $28 per hour based on qualifications.
