REGISTERED NURSE-Home Based Primary Care Clinical Coordinator

HBPC (Home Based Primary Care), aligned under Geriatrics, is recruiting for a full time RN for the newly approved position of Clinical Nurse Coordinator/ HBPC.
The Home Based Primary Care (HBPC) Clinical Nurse Coordinator is a registered nurse who demonstrates managerial and leadership skills in this supervisory role.
Under the direction of the HBPC Program Director the incumbent is responsible for delegated management and staff functions.
This person may be based out of West Haven or Newington depending on space.
Flexibility to travel to Newington or West Haven campuses depending on primary office location and HBPC sites at CBOCs located in Winsted, Waterbury and New London is required.
This position reports to the Director/HBPC.
The incumbent participates in assessing the nursing care provided in his/her area; responsible for the management and evaluation of patient care services within an assigned clinical setting and collaborates with HBPC Interdisciplinary staff to identify development/competency needs of the program.
Must be highly flexible, possess the highest degree of integrity, uphold professional, clinical standards and practice and display loyalty to the mission and values of the VACT Healthcare System.
The HBPC Clinical Nurse Coordinator interprets the philosophy and goals of Nursing Service to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop sound relationships with patients, their caregivers and other professional and supportive personnel who provide client care.
Duties include but are not limited to:
Coordinates daily logistics regarding program's turbulence (admissions, discharges, and transfers in/out), triage and patient care needsProvides staff with educational and clinical oversight to increase patient visit productivityMonitors and contributes to individual patient care and discharge planning activities Directly counsels staff not meeting productivity performance measuresReviews patient panels for short-and long term program objectivesEstablishes priorities for patient care specific issues based on changes in patient population and availability of resourcesAdheres to professional and legal standards and maintain confidentiality of patient and staff informationMaintains clinical expertise consistent with patient population requirements and current nursing and healthcare practiceProvides input into financial management activities including operational budgetary decisions and cost containment Assesses available resources to provide patient care and recommend changes in staffing based on program's needs and costServes as a role model for staff by demonstrating clinical expertise when providing direct patient carePractices according to approved policies, procedures, protocols and practice guidelinesProvides supervision of staff.
Assists staff members through more complex procedures and emergent clinical situationsAssists in development, interpretations, and implementation of the philosophy, procedures, and standards of careAssists the Program Director with the establishment and promotion of program goals and activities, including evaluation toward attainment of such goalsDevelops, implements and evaluates patient/family health education activities for the patient population with regard to the condition and age of the patientsReviews program's staffing schedules to ensure adequate coverage.
Anticipates staffing needs based on patient acuity and staff skill level on an ongoing basisAssesses patient care to ensure that patient assignment system reflects patient care needs and staff capabilitiesParticipates in initiation, development, and implementation of new policies and proceduresIdentifies and communicates to Program Director regarding program services and standards.
Utilizes problem solving skills to facilitate alternatives and solutionsParticipates in HBPC and/or hospital nursing committeesParticipates in special projects and assignments as requiredMaintains professional behavior according to the ANA Code of Ethics and Nursing Service Ethics policyAssists in the quarterly collection and analysis of data leading to HBPC quality performance improvements Work Schedule:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
