This assignment carries primary responsibility forproviding in-home occupational therapy assessments, and the planning, and administration and management of rehabilitation in the home for patients of the North Florida/South Georgia Veterans Health System (NF/SG VHS) Home BasedPrimary Care Program (HBPC).
Additional responsibilities, as HBPC duties allow,may include assisting in-home assessments of profoundly disabled patients of NF/SG VHS who are not enrolled in HBPC.
The OT will provide these in-home services to patients with a wide variety of multiple, complex chronic illnesses and/or profound disabilities.
Duties to include:
o Develop, implement and evaluate therapeutic rehabilitation programs.
o Assess the goals, living skills, physical abilities, and related needs for each patient.
o Procure necessary equipment/adaptive devices for direct patient care services.
o Reassess patients' levels of functioning and home environment.
o Revising the treatment plan as indicated when patient has achieved maximum benefit from PM&RS.
o Evaluate the need for additional services.
o Initiate consultation with other professionals to provide additional needed services.
o Evaluate the home for structural or positional changes in order to promote the patient's safety and o independence.
o Complete assessment and documentation required by performance measures applicable to the OT's role.
o Serve as content expert in ordering complexand specialized medical equipment.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
