_ABOUT THE POSITION_
We are currently seeking a dynamic Intensive Care Manager to join our team at our office in Rocky Hill CT. The ICM clinician is responsible for applying specialized care management techniques in the clinical management of intensive, chronic, or complex cases and when an individual experiences barriers to recovery.
_Position
Responsibilities:_
_Services as a resource to members and providers telephonically for information or clinical process via the Call Center protocols. The majority of the ICM work is done telephonically._
_Collaborates with the Department of Children and Families and the Department of Social Services as necessary to establish treatment opportunities for Members_
_Participates in multidisciplinary team reviews on a regular basis to review existing care plans to ensure they adequately address the behavioral health needs including complex behavioral health issues_
_Participates in a Geo-Team approach to care management to enhance Member treatment by identifying resources, access to care, identifying barriers to care and gaps in services_
_Responsible for the intensive care management of designated Members who meet ICM criteria in a designated local service area; will at times work out of a local area office when space is available_
_Responsible for managing a written ICM plan which reflects the Member's needs. Existing care plans may be reviewed with providers._
_Position Requirements:_
Education _: This position requires a Master's Degree and independently licensed in one or more of the following: Social Work-LCSW, Licensed Professional Counselor-LPC, License Drug and Alcohol Counselor - LADC, Marriage and Family Therapist-LMFT, or Registered Nurse-RN._
Licensure : _Connecticut licensure _
Relevant Work
Experience: _Five (5) years' experience in the behavioral health field with two (2) years post master's degree experience in the delivery of social work/counseling services in a behavioral health care or home health setting is required. Knowledge of local community resources is essential._
Knowledge, Skills & Abilities :
_Excellent communication skills, both verbal and written_
_ Working knowledge of care management principles and practices, cost containment principles_
_Excellent PC skills, analytical and problem solving skills needed._
