Full Time. Reporting directly to VP for Client Relations, work in collaboration with physicians, staff and other healthcare professionals to promote timely access to needed care, comprehension and continuity of care, and the enhancement of patient and family well-being. Coordinate appropriate resources to facilitate and ensure the patients progress through the continuum of care. Actively participates in the quality review process and assures continual improvement of patient safety, clinical practice and quality patient care. Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained across the entire continuum of care. Responsible for assisting in the coordination of a wide range of self-management support and disease registry activities. Health Educators do not provide medical advice or work clinically within this role. Requirements: Masters degree in health education, nursing, or related field. In lieu of masters degree, will take a bachelors degree in health education, nursing, or related field, plus 5 years experience in health education or nursing.
