Assists in the collaboration, research, development, implementation, revision and reporting of the resource management program. Working with all appropriate members of the health care team, ensures coordination of patient care through the development of age-appropriate plans to address the physical and psychosocial needs in the discharge planning process. Provides patient education and arranges for post-hospitalization services in accordance with the interdisciplinary team's provision for care after discharge. Ensures that specific outcomes are achieved and that variances from the established and accepted parameters are evaluated and addressed as necessary. Fill in on schedule as needed on a PRN basis including weekends.
Qualifications:
Bachelor's Degree in Social Work required.
MSW preferred.
One year experience in an acute healthcare setting is required.
Current state of Mississippi licensure as a LSW is required.
CPR certification required (or within 90 days of hire).
Job: Social ServicesEstimated Salary: $20 to $28 per hour based on qualifications.
