Job Description: Facilitates processes on a daily basis to expedite patient''s care analyzing variations from predicted utilization. Coordinates input from all health professionals, conducts assessment of patient and family needs and formulates a documented discharge plan assuring continuity of care. Facilitates appropriate use of hospital resources by reviewing all patients admitted to the hospital and evaluating appropriateness of admission and continued stay using approved criteria. Facilitates appropriate reimbursement through ongoing communication to insurance company.
Effective December 9, 2013 LVHN will begin testing all newly hired employees for nicotine and nicotine metabolites at the time of the pre-placement physical assessment to verify their response to the nicotine use question in the pre-placement medical form. The use of nicotine is _not_ an automatic disqualifier of employment with LVHN.
Minimum Requirements: 1. Interpersonal skills necessary to negotiate with families, patients, physicians, and third party payers for optimum plan. 2. Excellent communication (oral and written) skills required to ensure patient, family and team satisfaction. 3. Ability to work independently, setting priorities to coordinate care plans efficiently under constraints of managed care. 4. Knowledge of Community Agencies required to provide patients and families with appropriate choices and options for post hospital care or prevention of admission. BSN, BSW, or Bachelor''s in health care related field. Will consider Associate Degree RN on a case by case basis.
Influenza vaccination will be required for all personnel working within LVHN, who have patient contact, unless medically contraindicated or due to a bonafide religious belief or practice.
Preferred
Qualifications: 1. Two years previous Utilization Review, Discharge Planning, or Case Management experience. 2. Knowledge and application of InterQual Criteria to clinical reviews. 3. Case Management Certification. 2 yrs experience in health care related setting ACM or CCM, LSW, Masters in Social Work (MSW) Degree from a program accredited by the council on Social Work Education, MSN preferred
