POSITION
Summary:
Delivers home health care to client/patients of all ages and families whose needs range from basic to complex and are rapidly changing by utilizing the nursing process. Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being.
PATIENT CARE/QUALITY:
Assesses patients physical and psychosocial status and the home environment.
Plans and implements patient care in collaboration with the physician.
Evaluates and updates appropriateness of care plan and physician orders.
Communications appropriate information to patients, peers, managers, physicians, department, and other agencies involved.
Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
Qualifications:
Education:Graduate of an accredited school of nursing; BSN or equivalent preferred.
Experience:Equivalent of one year in a staff nurse position in acute care setting preferred.
Licensure/Certification:Current Licensure in State of New Hampshire as a professional nurse.
Valid Driver s License:Required to travel throughout southern New Hampshire to provide home health care service and other job-related activities.A motor vehicle background report will be conducted.
Other:Proof of auto mobile insurance required.
