Job Description Job Summary/Purpose: Coordinates and directs the referral activities from healthcare facilities to home care by identifying/determining appropriateness of care, gathering medical and financial data, meeting with patients and families, and working with Care Management, physicians and referral sources. Responsible for safe and effective transfer from facility to home. Referrals include Home Health, Hospice, IV and Home Medical and Respiratory Equipment.
Minimum
Qualifications:
Education: RN license required.
Experience: Minimum of 3 years of home care, discharge planning or case managementexperience required.

Day shift.
Must have recent, current RN experience
