Description
The Clinical Integration Specialist (CIS) is a registered professional nurse with education, knowledge and experience to support and facilitate a patient centered quality care coordination model at various points of entry. The CIS role focuses on criteria for admission status assignment, primary and specialty care physician collaboration and communication, medical necessity for procedures, present on admission conditions and care coordination aspects of population health. The CIS evaluates cases to determine the utilization and quality aspects of admission as it occurs in the acute care setting. The CIS facilitates modifications to clinical documentation through extensive concurrent interaction with physicians, nursing and ancillary staff to support appropriate utilization through capture of documentation reflecting appropriate clinical severity. Supports timely, accurate, and complete documentation of clinical information used for measuring hospital initiatives and reporting physician and hospital outcomes. Adheres to regulatory guidelines to ensure medical record integrity. Communicates and collaborates with the inpatient clinical quality team and other care support departments to provide baseline level utilization and clinical quality assessment information in accordance with the patient's plan of care. Provides ad hoc support to inpatient units to ensure compliance with all components of the clinical quality integration model. The CIS nurse role models Excellence for Life principals and standards of behaviors.
Requirements
Current Ohio RN Licensure required. Bachelor's Degree from an accredited School required, Master's Degree preferred. CPHQ preferred. Minimum five years clinical experience required, critical care or ED Experience preferred. Two years' experience in case management/clinical outcomes and/or clinical documentation specialist required. Knowledge of federal, state, Healthcare Facilities Accreditation Program standards, utilization and quality improvement regulations and their application needed.
