Clinical Social Worker
Lorien Columbia seeks an experienced Clinical Social Worker with a background in long term care case management.
The ideal candidate must possess critical thinking skills, patience and emotional strength.
You must also be driven in pursuing the social work role and developing your clinical skills.
As a Social Worker, you should be comfortable helping patients adapt and must collaborate daily with our nursing units and other departments on behalf of our patients.
Strong written / verbal communication skills are necessary and you must enjoy being a part of a team.
Responsibilities Evaluate existing, new and prospective patients based on their needs, desires and limitations Assist in the admissions and the discharge process as needed.
Assist in completing advanced directives, referrals to services such as psych and hospice.
Assist with educating and making referrals to community resources.
Long term case management for our Korean speaking unit of around 5 patients.
Qualifications Master's Degree in Social Work 5
years clinical social work Valid and current LCSW or LMSW license, preferred Excellent listening and communication skills Bilingual preferred Experience with EMRs 'min_experience':

Estimated Salary: $20 to $28 per hour based on qualifications.
