Summary:
This position will be held by social worker's in training, who have successfully completed a student experience at SEMC. In this position, they will provide a cost-effective technical support for the permanent social work staff. Under the guidance and direction of the licensed Social Worker, the social work technician participates in daily routine discharge activities, assisting in the acquisition of the patient admission assessment, participating in the discharge plan, following thru with the implementation of the plan and the acquisition of the appropriate services. In collaboration with the licensed social worker, evaluates the progress made and the finalization of the discharge plan. Within the scope of their ability, the SW Technician provides emotional and financial well-being of the patient's on their assignment.
Responsiblities:
Under the supervision of the licensed social work discharge planner/care coordinator (LCSW or LICSW):
Reviews the daily patient census with licensed social worker and nurse case manager
Identifies the unit patient admission and discharge care needs with licensed social worker
Understands LOS and safe discharge, their importance and the SW technician's role in achieving these goals
Obtains assignment for the day and completes in a timely and quality of care manner
Collaborates in the completion of the social work admission/discharge assessment and documents in Allscripts
Follows up, as appropriate with home care services and/or nursing home, rehab facilities per patient/family choice
Obtains the Medicare Important Message, as well as other compliance and quality metric data collection/report completion
Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
Commits to recognize and respect cultural diversity for all customers (internal and external).
Communicates effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed.
Performs other duties as assigned
Qualifications:
Enrolled in an undergraduate or graduate social work or related field, no specific licensure required.
Minimum of 3 years of undergraduate education or in graduate school with major in Social Work, Health Policy, or related field.
Completed successfully a social work student experience at SEMC, as a social work student assistant in all elements of the discharge planning role.
Strong interest in the health care setting and health policy
Demonstrates strong customer service skills and has the basic ability to be a patient advocate
Ability to follow direction from the licensed social worker and is open to recommendations for improvements, growth and development in this role
Demonstrates a level of emotional intelligence
Ability to follow thru and complete assignments in a timely manner and document clearly, concisely and completely
Available to pick-up work shifts on week days, weekends and holidays
Basic Allscripts and Meditech experience obtained during their student experience at SEMC
