Polytrauma Pain Management RN Case Manager

The Polytrauma/Pain Management RN Case Manager is responsible and accountable to the ACNS/Ambulatory Care and the Chief of the Division of Rehabilitative Services.
The Polytrauma/Pain Management RN Case Manager are responsible for the overall direction of team members to achieve desirable patient outcomes through coordinated case management.
Patient encounters may occur through traditional clinic visits, via telephone, or virtually, using Clinic Based Telehealth (CBT).
The Polytrauma/Pain Management RN Case Manager assists with the development, assessment, implementation, and monitoring/evaluation of comprehensive plans of care to ensure efficient, effective, and coordinated care in meeting and promoting standards of wellness.
He/she will provide direct and indirect nursing care to patients.
He/she coordinates an interdisciplinary approach to assure physical, psycho-social, and environmental needs and resource delivery to achieve patient goals, The Polytrauma/Pain Management RN Case Manager relates in an open, friendly manner, shows interest in others, and initiates and develops interdisciplinary relationships for a board spectrum of resources.
He/she acts as an advocate seeking and coordinating solutions for self and members of the team.
The Polytrauma/Pain Management RN Case Manager adheres to established policies, standards of care, and standards of practice; demonstrates leadership when providing nursing care; and serves as a role model when assigned as a Polytrauma/Pain Management RN Case Manager nurse under the supervision of the Nurse Manager or designee.
The Polytrauma RN Case Manager is responsible for coordination of health care for a defined patient population identified as combat wounded.
Patients receive goal-oriented, coordinated care addressing holistic needs with the aim of preventing disease complications, maximizing functional ability, and quality of life.
Polytrauma describes the pattern of injuries that typically results from OEF/OIF combat conditions.
The outcomes for combat wounded OEF/OIF veterans with severe injuries may include but not limited to one or more of the listed below:
o Traumatic Brain Injury (TBI) o Upper limb amputation o Lower limb amputation o Blindness o Burns o Post-traumatic stress disorder The Pain Management RN Case Manager serves as a clinical expert and liaison in coordinating pain management clinic services.
He/she will provide direct and indirect nursing care to patients with a variety of pain-related conditions utilizing the nursing process, communication, and problem solving skills to deliver quality care.
The Pain Management RN Case Manager will be responsible for maintaining knowledge of pain management standards of care and practice and of VHA, Joint Commission and other applicable pain management guidelines.
Acquisition of additional knowledge and skills needed to provide or assist with interventions and/or complementary and alternative approaches to pain management may be required to fulfill clinical duties and accomplish desired outcomes.
Services within the Pain Clinic are provided in individual or group face-to-face contact, telephonic and/or virtually through tele-pain modalities.
Additional role responsibilities and accountabilities include the following:
o Successfully pass and maintain ANCC Board Certification for Pain Management Nursing within 18 months of hire.
o Complete all required Telehealth training within six months of hire and renew annually, or, as required.

Estimated Salary: $20 to $28 per hour based on qualifications.
