Inpatient Clinical Pharmacist (Overnight Pharmacist)

The clinical pharmacist functions independently with prescribing privileges under a scope of practice in the acute care setting.
The clinical pharmacist is fully responsible for planning, developing, and maintaining highly innovate and progressive clinical pharmacy programs in their assigned area by implementing rational cost effective drug therapy to improve patient outcomes.
This includes, but is not limited to, the following:
designing, implementing, assessing, monitoring and documenting therapeutic plans; helping achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams; and ordering laboratory tests to help determine efficiency and toxicity of medication therapy.
The clinical pharmacists assists with clinical education activities, provides drug information, contributes to medication use evaluation (MUE) activities, and provides support to the Medication Management Committee (MMC).
The clinical pharmacist must be capable of using professional judgment and clinical expertise to solve complete problems and to provide clinical information to VA Connecticut Healthcare providers and Veterans.
Distributive responsibilities will be incurred in this position when appropriate.
Other duties include but or not limited to:
o Designs dosage regimens from current pharmacokinetics principles and specific patient parameters, and effectively communicates recommendations to the patient's provider and documents activity in the patient's medical record.
o In consultation with other providers, writes orders for medication and laboratory orders in the Computerized Patient Record System (CPRS) under a scope of practice.
The Clinical pharmacist specialist will independently manage patients on medications requiring intensive therapeutic drug monitoring (i.
e.
vancomycin, aminoglycosides, warfarin) to maximize patients response and minimize drug interactions and adverse drug effects.
o Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications.
This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking.
o Serves as an authoritative information source on drugs and their utilization in therapy.
o Serves as an expert in clinical pharmacy practice to the acute care-staff, responding to requests from practicing clinicians regarding difficult therapeutic problems with a high degree of complexity.
o Ability to perform all vault duties when processing controlled substances for inpatient or outpatient dispensing.
Complies with all VHA and DEA policies and regulations to assure strict drug accountability.
Ability to assure strict accountability of high risk cost drugs which are requires to be maintained via a perpetual inventory.
o Ability to respond to a disaster should the drug cache need to be activated.
Knowledge of emergency preparedness requirements and the types of drugs used to treat specific types of exposure related to a disaster.
o Knowledge of simple procurement requirements to assure that a drug can be acquired during the evening, weekend and holidays hours.
o Ability to prepare sterile IV products, including but not limited to chemotherapeutic agents.
Knowledge of all USP Chapter 797 requirements for the preparation of sterile IV products.
Knowledge of Intellidose or other software technology used for the management of chemotherapeutic agent dosing.
o Participates in the development and enforcement of Pharmacy and Health System policies and procedures when appropriate.
o The incumbent will assist in the dissemination and enforcement of guidelines established by the Medication Management committee regarding the use of formulary, non-formulary, and restricted drugs.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
