The Case Manager/Social Worker is responsible for assisting in coordination, consultation and facilitation of episodic patient care. The Case Manager/Social Worker assists the patient care team by: managing an assigned caseload of patients; assessing patient/family needs; use structure and process standards, clinical pathways or care plans to prioritize and communicate care requirements; determine the types and amount of clinical resources needed to meet patient/family needs; provide timely and appropriate interdisciplinary referrals; and assist with the measurement of selected patient outcomes. The Case Manager/Social Worker performs utilization review, clinical document analysis working in close collaboration with hospital staff as necessary to assure the most effective utilization of services. The Case Manager/Social Worker acts as a communication link between the patient, nurses, physicians, families/significant others, ancillary departments, extended care facilities, community resources and insurance companies. The Social Worker is also active in all initiatives involving social services (Sr. ED, Palliative Care, etc.) and those deemed necessary. Oregon Medical Social Services license required. One to two years experience required. BLS is required.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
