POSITION SUMMARY
Coordinates the development and accurate completion of the Resident Assessment Instrument (RAI) Minimum Data Set (MDS) in accordance with Federal and State regulations and Bon Secours policy and procedures.
JOB REQUIREMENTS
1. RN or LPN currently licensed with the Virginia State Board of Nursing.
2. Minimum of 2 years experience in skilled nursing facility
3. Knowledgeable of MDS process and assessment completion
4. Demonstrates general competency in providing Age Appropriate Care as indicated by the ability to plan, provide
and evaluate care based on the physiological, psychosocial, and intellectual and safety developmental needs of
the resident.
5. Coordinates RAI process and care planning per regulatory guidelines.
6. Certified Assessment Coordinator or completes MDS 3.0 training and obtains certification within 6 months.
