Provides leadership and consultation to patients, families, nurses and other health care providers to effect patient care outcomes. Utilizing established guidelines, assesses patients who have completed active therapy and are being followed on a regular basis for any evidence of disease activity. Evaluates patients who develop acute problems when physician is not available. Prescribes medications according to treatment plan, and refers patients according to guidelines. Coordinates the day-to-day operations and fulfills the responsibilities of the Clinical Coordinator of the Department. Coordinates the case management of patients being followed by the department. MS in Clinical Nursing Specialty recommended & N.P. certificate required. Minimum 5 yrs. recent hospital experience & 2 yrs.as advance practice nurse recommended; demonstrated leadership skill and expert practice. Current licensure as an APRN by the State of Vermont. Credentialed by the BMH Medical Staff; DEA license required.Estimated Salary: $20 to $28 per hour based on qualifications.
