The Social Services Coordinator assists patients and families in restoring social, emotional, and financial functions related to illness and disability using a wide variety of techniques and resources. S/he works in collaboration with Clinical Resource Coordinators with primary responsibilities related to family dynamics, relationships, adjustment problems and complicated community referrals. S/he may interface with patients and families at any point on the care continuum.
1. Assesses the psychosocial status related to the patient''s ability to cope with daily living and to make adjustments required by medical condition as requested by clinical resource coordinator, nurse, and physician or as required by regulatory agencies.
2. Maintains current knowledge of community and other resource networks for planning/provision to meet patient''s services needs.
3. Directs and supports patients/families toward obtaining and utilizing financial resources to meet current and continuing health care needs and support proactive transition/discharge planning goals.
4. Provides short-term support services for selected patients/groups and basic crisis interventions as requested by clinical resource coordinator, nurse, physician or as required by regulatory agencies.
5. Completes all clinical record documentation as required by facility procedures in a timely manner.

Qualifications:
Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW) with Master's degree in a social services license eligible field; MSW and 2 years acute care setting and/or licensure preferred.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
