OverviewAtlantic Health System is seeking a Full Time Resp. Care Pract. for Newton Medical CenterResponsibilities
COPD Educator functions to include:
Identification of all COPD patients in both HMC and NMC. Education of patient and family with Established Gold Standards. Education of staff to supply the required COPD support in Disease knowledge, Medication management and use, Smoking cessation, and Oxygen Therapy. Smoking Cessation Program management. PFT, Pulmonary Rehabilitation, support with basic Respiratory Floor care as needed. Follow up phone support to all discharged patients, one day, three days, and 10 days post discharge. Support to local SNF. Direct interaction with COPD Nurse Navigator. Serve on AHS COPD re-admission committee.
Qualifications
Requires -
Graduate of an AMA approved school of Respiratory Care
CRT, or RRT, CTTS, BLS
