Florida Hospital Wesley Chapel has provided faith-based, whole person care to the community since it opened in 2012. Every detail of our 83-bed facility is purposefully designed to create an environment of healing for mind, body and spirit. Our hospital is equipped with state-of-the-art equipment from outpatient services to our surgical suites and inpatient care. In 2013, we also opened a 50,000-square foot Health and Wellness Center that includes an indoor track, group exercise areas, and an aquatics center with a lap pool to help community members achieve optimal health. In August 2015, the hospital began construction on am expansion that will open in December of 2016, significantly increasing the number of inpatient beds, surgical suites, and emergency department rooms.
You will be responsible for:
Evaluates all referrals and identifies discharge planning needs in accordance with established criteria.
Communicates with the patient/family and healthcare team to assess and identify individual discharge needs and desires, including those specific to the age of the patient being served (neonate, child, adult, and geriatric).
Assists the patient/family and healthcare team in implementing the discharge plan.
Assists the patient/family in investigating eligibility and applying for Medicaid or SSI through Meds by referring to the financial counselor.
Utilizes community resources and refers the patient /family to appropriate agencies and services when necessary.
Communicates with extended care facilities to assure bed availability and arranges placement.
Arranges transportation for emergent and non-emergent transfers via ambulance, air ambulance, helicopter, non-emergency transport and commercial airlines.
Arranges care at home including, necessary equipment and personnel prior to patient discharge.
Documents patient interviews, evaluations, recommendations and summaries in the medical record.
Documents the discharge disposition utilizing Care Management.
Assists the Director of Case Management in developing and monitoring criteria and implementing the Performance Improvement (PI) Program for the Case Management Department.
Reports problems or concerns regarding the quality and appropriateness of patient care delivered to the immediate supervisor at the time of identification.
Demonstrates the ability to evaluate the effectiveness of Case Management/Social.
Services utilizing the PI.
Process to evaluate patient outcomes.
Observes all established policies and procedures throughout the daily work routine.
Responds to all disaster situations and implements the disaster plan.
Performs in a calm manner during crisis or emergency situations.
Reports observed deviations from established policies and procedures and standards of care to the immediate supervisor in a confidential manner.
Protects the patient's rights; maintains confidentiality of all patient related information.
Demonstrates the understanding and ability to execute the role of Social Worker, remaining up to date on Changes in regulations, policies and procedures.
Applies advanced problem solving techniques in planning, assessing, implementing and evaluation of patient Discharge needs.
Understands the Case Management/Social Services philosophy and principles of interdisciplinary team management and collaborative practice.
Utilizes tools and resources to develop a comprehensive plan of care.
Demonstrates an understanding of planning and goal development techniques.
Intervenes when variances occur in the patient's individualized discharge plan.
Demonstrates the team concept; works in a cooperative manner with co-workers and supervisory staff.
Utilizes the appropriate chain of command to resolve conflicts.
Assists with orientation of new employees and provides learning experience for affiliate students.
Contributes to the smooth operation of the Clinical Case Management Department by performing other related duties as assigned by the Director of Case Management such as but not limited to:
Demonstrates flexibility as patient needs and census changes.
Assist with discharge planning needs in the ER and Short Stay Procedure Unit.
Performs other tasks as assigned which are within the employee's capability.
Qualifications
What you will need:
Minimum required Bachelor's Degree in Social Work. Preferred Clinically Licensed Social Worker or Masters in Social Work.
2 years of experience in Social Work in a health care setting.
Bachelor's Degree in Social Work.
Clinically Licensed Social Worker or Masters in Social Work preferred.
Stoke Certification - 1 Hour CEU.
Hip and Knee Joint replacement Certification 1 hour CEU.
State licensure Social Worker preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
