PD Social Worker (MSW)

OverviewChilton Medical Center, a member of Atlantic Health System, is seeking a Per-Diem Social Worker working primarily 8:30 AM - 4:30 PM, including one weekend per month.Responsibilities
Essential Job Functions:
Respond to cases as assigned to coordinate and support discharge planning, placement, resource and referrals, support for both patient and families.
Actively participates in multi-disciplinary rounds
Works closely with RN Care Manager, Physicians and RN's to support the plan of care.
Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
Candidate will demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job.
Qualifications
Education:
Masters in Social Work Required
Required License:
Current LSW or LCSW Social work license Required
Related
Experience:
Ability to work with an electronic medical record, Meditech/EPIC experience a plus.
Excellent verbal and written skills, willingness to work closely in a Team oriented environment.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
