Hospice VNA Every Other Weekend On Call

The On-Call Nurse is responsible for the provision of holistic skilled hospice nursing assessment, intervention and supportive care. VNA traige as well. Ensures continuous coverage for incoming calls after office hours, therefore, is available by phone to patients/caregivers and staff via switchboard operator. Triages patient/caregiver related phone calls, provides telephone education and guidance, assess need for visit, and conducts home visits as needed for symptom management and pronouncements.
RN licensure and Hospice and Home Care experience required.
