Overview
Determines patient eligibility, plans, provides, delivers or supervises coordination of services. Maintains and adheres to governmental regulations. Ensures optimum quality of care consistent with Agency practice in relation to the Agency goals and objectives.
Responsibilities
Determines patient eligibility as established by the program criteria, which may include a home visit for assessment purposes.
Submits documentation for services provided according to Agency protocol.
Collaborates with physician/patient/family, insurance carrier to establish and revise as necessary patients' coordinated plan of care.
May provide nursing care according to the prescribed treatment plan.
Ensures cost effective provision of services.
Provides supervision and instructions to ancillary staff.
Documents accurately the patient care and services rendered according to Agency protocol.
Participates in patient care conferences as indicated.
Keeps informed of any changes within the program that would affect patient care.
Keeps informed of community resources and abreast of changes in home care legislation.
Assists in orientation and/of guidance of new staff members (professional and non-professional, nursing students and volunteers).
Qualifications
A Bachelors Degree in Nursing and two (2) years of nursing experience including one (1) year in a LTHHCP or CHHA setting; or Associates degree in nursing and three (3) years of nursing experience including one (1) year in a LTHHCP or CHHA setting; or a satisfactory combination of education and experience.
Current license to practice as a Registered Professional Nurse in New York State.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
