Registered Dietitian
Job Description:
To see referred patients in their home, assess their nutritional needs, and formulate a plan of care to promote their health.
Counsels individuals and families in nutrition principles, diet, food selection, and economics
Acts as a resource for all staff in assessing and instructing clients and others about appropriate nutrition for Agency clients
Works to achieve favorable patient outcomes through appropriate case management
Job Requirements:
Bachelor's Degree with major in foods, nutrition, food service or institution management, Nutrition or in Public Health Nutrition
Has completed a dietetic internship accredited by the American Dietetic Association or a dietetic traineeship approved by the American Dietetic Association
Or has one year of full-time, or a full-time equivalent, experience in nutrition and/or food service management in a health care setting; or has a master's degree plus six months of full-time, or full-time equivalent, experience in nutrition and/or food service management in a health care setting.
Demonstrates effective communication skills
Demonstrates initiative and is able to function independently without direct supervision and as a member of a team
Must be flexible to meet patient care needs
Demonstrates the ability to initiate, accept, and adapt to change
Demonstrates the ability to drive to patient homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs
Coordinates services with other disciplines and community agencies to meet patient needs
Demonstrates ability to work in patient homes in less than ideal states of cleanliness
Demonstrates good organizational and time management skills to meet patient care needs and agency requirements
Must have current auto insurance at NY State minimum required levels
Must possess and maintain current NY driver's license
Proof of current professional liability insurance
