Job DescriptionThe Home Health Physical Therapist is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship by therapy program involving exercise, massage, heat, water, light, and electricity.
Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests.
Performing interdisciplinary case management and providing appropriate age-specific therapy services.
Primary AccountabilitiesPerforms complete assessment/evaluation including, but not limited to, home safety, pain, nutrition, fall risk and DME needs.
Develops/Implements a Plan of Care based on assessment findings and Physician order.
Assists with the orientation process of new associates and staff education on an as needed basisComplies with State and Federal regulations, JCAHO standards reference delivery of Home Care Services.
Evaluates patient outcomes, communicates progress, and changes in condition to physician.
Supervises the Home Health Aide no greater than every 14 days.
Supervises the Physical Therapy Assistant and ensures competency annually.
Documents alterations in the Plan of Care with Treatment Orders sent for signature to MD on the day the change occurs and progress towards goals.
Ensures appropriate utilization of visits and services for care ordered as noted through proper documentation completed within 2 days of the visit.
QualificationsGraduate of an accredited school of Physical Therapy (BachelorEUR(TM)s required, Doctorate preferred)Minimum of 1 year acute care experience.
Home Care experience preferredCriminal History Screening to include Level II FingerprintsComputer skills.
Current Florida Drivers License, automobile insurance and reliable transportation.
Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.
Current licensure in the State of Florida as a Physical Therapist.

Estimated Salary: $20 to $28 per hour based on qualifications.
