Position Description:
We are seeking a certified Physical Therapist for a Part-Time caseload in the Phoenixville, PA, area working with the geriatric population.
Qualifications:
o Must be a licensed/certified Physical Therapist in the state of Pennsylvania.
o Must have experience working with the geriatric population.
o Must have excellent references.
Responsibilities:
o Consultation and collaboration.
o Evidence-based decision-making and accountability.
o Provide initial and ongoing assessment to determine level of functioning.
o Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of patients' physical
and psychosocial functioning.
o Develop and revise the plan of care in consultation with the physician and other care-team members.
o Responsible for planning, evaluation, and treatment implementation of assigned caseload.
o Prepare clinical progress notes.
o Legal, ethical, and professional practices in compliance with federal and state mandates, as well as local policies concerning performance.
Estimated Salary: $20 to $28 per hour based on qualifications.
