RN/Social Worker Need IMMEDIATELY in Phoenix City AL
Delta Homecare is currently seeking Registered Nurses/Social Workers (Case Management experience a PLUS) to take on patients. The RN/Social Workers duties will require the individual to update Delta Homecare on the progress or decline in patient's status/health, help the patient maintain a healthy lifestyle by suggesting activities based on individual patients needs, assist the patient in scheduling doctor's appointments and obtaining information on patients medication refill, maintaining accurate information for Delta Homecare's portfolio for each individual patient. Qualifications:
RN License-Social Worker Degree/Transcript
Minimum of two years experience (at least one year of which must be in Geriatrics)
Reliable transportation
