Care Manager Rn and Lvns
Enterprise Health Staffing LLC is seeking dynamic nurses RNs, LPNs and LVNs to work in Comanche TX and surrounding areas.
Our Care Managers (licensed RNs / LPNs / LVNs) act as advocates and navigators to assist members/patients with accessing benefits and resources. They are the client's coach for resolution regarding disease management, self-care, medication reconciliation (to help avoid medication confusion), and identification of gaps or barriers to care (without providing direct care themselves).
The Care Manager contacts the client within 24 hours of receiving the assignment to explain the in-home care management program and to schedule the first visit. Upon the first visit, the Care Manager shares program documents with the member. Weekly face to face meetings are scheduled for the convenience of both the Care Manager and the member. As the member's needs change, services will be modified to meet an appropriate level of care.
Requirements:
At least 1 year experience as being a nurse. Home Health experience required:
. Ability to use computer for all nursing notation. Strong critical thinking skills and work independently.
