RN Case Manager
Home Health Agency is seeking a full-time RN Case Manager for Cedar Park and surrounding areas. Position will be Monday-Friday. Home Health Experience preferred. We offer competitive salary, Paid Time Off (PTO), health, dental, vision and life insurance.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide skilled nursing care to patients on a recurring basis in their place of residence.
Provide services in accordance with the patient's care plan, in coordination with other members of the health care team, and in compliance with all regulatory guidelines.
Evaluate the patient, family members, and home environment to determine required services; continually assess the patient's needs.
Review and provide supervision for each patient's care plan; stay informed of services, diagnoses, medications, procedures, goals and progress.
Teach, supervise and counsel the patient and family members regarding the nursing care needs.
Coordinate and supervise care provided by LVN and paraprofessionals assigned to the patient and assure continuity of quality patient care.
Consult with the attending physician regarding any changes to the plan of treatment under direct supervision of the Director of Nursing.
Coordinate transfer and discharge of patient from agency services as indicated by the physician.
Perform on-call responsibilities and provide on-call services to patients/families as assigned.
Submit all patient notes and/or visit records on a timely basis.
Participate in interdisciplinary team conferences.
Attend on-the-job and in-service training.
EDUCATIONAL EXPERIENCE AND REQUIREMENTS:
Current Texas State license as a Registered Nurse
CPR certification
Minimum of one to three years of experience in home health
Supervisory experience preferred
Electronic documentation experience
Excellent verbal and written communication skills
Ability to work effectively with others
Ability to exercise initiative and independent judgment
Valid Texas driver's license / auto insurance
Reliable transportation
Estimated Salary: $20 to $28 per hour based on qualifications.
