Case Management Assistant

DEFINITION :
The Case Management responsibilities including promoting quality patient care to ensure effective, high quality and cost efficient results in moving patients through the continuum.
Case management assistant requires a high level of independent sound judgment, creativity and organizational/priority settings abilities.
It also requires the coordination of both internal and external health care team members to ensure optimal continuity of care for patients of all ages (neonates through geriatrics) as they move through the continuum of care.

Non-Exempt KNOWLEDGE:
Working knowledge of Title 22, Joint Commission and Medicare requirements regarding utilization review and discharge planning in an acute care facility preferred.
SKILLS:
Ability to work within a team environment with staff of varying licenses, abilities, and responsibilities, including physicians, nurses, technicians, clerical and support staff.
Ability to deal objectively with a wide variety of people and situations.
Ability to work independently and be self-directed.
Demonstrates responsibility and accountability for problem-solving and clinical decision-making and accepts responsibility for own actions and decisions.
Discretion, excellent communication skills, both verbal and written; outstanding interpersonal communication skills.
Working knowledge of medical terminology
Experience:
Two recent acute care experiences preferred as a Certified Nursing Assistant, Medical Assistant, or Unit Secretary.
Recent discharge planning experience preferred.
LICENSE:
High School Diploma or higher.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
