Community Case Manager - Adults
Provides support with a rehabilitation focus to adult clients. Assists clients in discovering and maintain valued roles in the community. Interfaces within the Agency to ensure optimum quality and comprehensiveness of services.
Brokers service and service links with outside agencies. Assists clients to maintain a community living by teaching activities of daily living and personal care; promotes independent and productive habits to clients in the community.
Also develops other programs and activities, which promote the introduction to and the integration of the client into the community.
Requires a Bachelor's degree in Human Services.
Estimated Salary: $20 to $28 per hour based on qualifications.
