Nurse Practitioner
RESPONSIBILITY:
The Nurse Practitioner (NP) works autonomously but in collaboration with the primary care physician(s) in the delivery of primary medical care in the home. The NP provides medical services by managing acute and chronic conditions and developing treatment plans for patients at home and other resident settings which may include: assisted living, independent living, and senior housing. The NP performs necessary teaching on chronic disease management. All documentation is performed through an electronic medical record. The NP works closely with their practice manager to coordinate patient care and expand the practice through referrals.
Qualifications:
Graduate of an approved Nurse Practitioner Program, with state licensure and certification in specialty and prescriptive authority from state, or graduate of a licensed program that meets the eligibility requirement of
state and federal licensure regulations.
Previous experience in geriatric primary care preferred.
Knowledge of Medicare reimbursement and coding for all levels of service - home, assisted living, etc., and other regulatory requirements.
Excellent oral and written communication skills, experienced working in a collaborative care team environment and functioning autonomously with minimal supervision. Bilingual is a plus.
Current CPR certification.
Experience with an Electronic Medical Record is preferred.
Computer literacy is a must.
Ability to work in a smoke-free environment.
