Manages each residents medical file and is responsible for coordinating all medical, dental, vision, and psychological appointments. Case manager is responsible for communicating all necessary medical changes to the appropriate family, program nurse, and staff members for the resident.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Schedules all client medical appointments including doctor, dentist, eye, psychiatric, neurological, etc. as needed.
2. Assures each client gets his/her respective appointments.
3. Contacts CLF nurse regarding all medical changes.
4. Serves as a member of the Behavior Management Team attending all meetings.
5. Communicates all medical changes to Habilitation Aides (DSP).
6. Maintains medical section of the medical file.
7. Keeps medical area clean and tidy.
8. Keeps monthly documentation up to date and organized.
9. Works on a one to one basis with residents in regards to grooming, housekeeping, personal hygiene, etc.
10. Oversees client self-administration of medicine under direction of nurse.
11. Works closely with nurse to ensure that monthly vital signs and weights are documented and organized.
12. Communicates any/all problems or concerns to the Program Director.
13. Works all scheduled hours which may require working more than 40 hours per week.
14. Perform other duties as assigned by the Program Director.
QUALIFICATIONS

Must possess exceptional communication and interpersonal skills
Education and/or Experience
Minimum high school diploma or GED.
Minimum two year workings with developmentally disabled adults.
Direct Support Person certified
Medical knowledge
Good communication and organizational skills.
Ability to relate positively to residents, families, co-workers, physicians, and community.
Hold valid drivers license with ability to get JO-5 restriction.
Certified, or willing to be certified, in CPR, and First Aid.
Ability to be flexible with work schedule when needed.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
