The Admission Coordinator Nurse qualifications must be as follows:
Current, active license to practice as an LPN or RN.
Minimum of 1 year experience in home health and hospice preferred.
Provide excellent customer service for clients, referral sources and internal staff.
Ability to manage multiple concurrent tasks and prioritize assignments
Excellent in both written and oral communication skills, including spelling and grammar.
Ability to handle stressful and delicate situations with tact, diplomacy, and attention to their importance.
Ability to assist with completing admission paperwork.
Manage calls from Haven field staff and act as liaison between physician office and Haven staff
Must represent Haven in a positive, professional and customer service focused agency.
Must have reliable transportation available during work hours.
Essential Job Functions/Responsibilities of the Admission Coordinator:
Receive referrals by phone or in person for designated department.
Provide explanation of the benefits to patients, families, and community contacts.
Assist referral sources with finding alternative services when patient is not a candidate for Haven.
Complete scheduling of initial nursing assessment and admission.
Completion of admission paperwork as needed.
Provide telephone and clerical support to other staff members as time allows.
Familiar with regulations to meet Medicare/Medicaid conditions of participation and accreditation requirements for all Haven Home Health programs and services and reflects the mission and philosophy of Haven Home Health.
Daily monitoring and management of pending admission lists and ensure smooth transition of clients onto services and provide the information field staff need to ensure good customer service.
Ensure pharmacy and DME needs for hospice patients are initiated properly.Estimated Salary: $20 to $28 per hour based on qualifications.
