Base Salary:
A fair market value first year base salary of $118,554 that is based on Practitioner working full-time (i.
e.
, 80 hours per bi-weekly pay period), payable in 26 equal installments.

Comprehensive
Benefits:
Practitioner will receive the following fringe benefits to the same extent as the Hospital makes them available from time to time to other Hospital-employed providers, consistent with Hospital's policies and plans:
generous annual accrual of paid time off (vacation, holidays, and short-term illness) and annual accrual of extended illness leave, generous allowance for continuing medical education, comprehensive health insurance (medical, vision, dental and orthodontia), generous pension plan, deferred compensation plan, travel and accident insurance, accidental death and dismemberment insurance, life and long-term disability insurance, discounted medications and meals, payroll deduct for Flexible Spending Account and AFLAC insurance, and generous allowance for professional expenses (license fees, dues, books and subscriptions).

Liability Insurance:
Liability insurance will be provided by Corporation with $1 Million/$3 Million coverage, and no tail.
4.
Moving Expenses:
Hospital will reimburse moving expenses, up to a maximum of $25,000.
5.
Education Loan Subsidy:
Hospital will reimburse up to $25,000 in loan debt principle paid by Practitioner during each 4-year contract period.
Accrued loan debt subsidy is subject to forfeiture if physician resigns prior to completing four years of employment.
Maximum lifetime benefit of $100,000 Oregon Rural Provider Tax Credit:
The State of Oregon offers an income tax credit of $5,000 for providers practicing in La Grande, which is equivalent to earning roughly another $9,000 in base salary that is not reflected in the numbers above.
Total compensation is based on fair market value, structured to avoid an excess benefit transaction.
