Service Coordinator - Housing

A Service Coordinator provides information, referrals & community connections to residents to maintain self-sufficiency; provides case management & hands on assistance to accessing resources as needed by individual residents & their families.
Essential Duties and
Responsibilities:
Establish & maintain positive working relationships with residents, their families & the community.
Assessment of health, psychological & social needs including the development of individually tailored case plan for services & periodic reassessment of the resident's situation.
Establishes linkages with all agencies & service providers in the community Establish & display on site community resource information for services Educates residents on serve availability, application procedures, client rights, etc.
providing advocacy as appropriate.
Encourages residents to build informal support networks with other residents, family, friends, & volunteers.
Maintain schedule flexibility as needed for resident/family meetings.
Coordinates wellness/education sessions to residents on subjects of interest a minimum of one time per month.
Exchanges information with on-site team including the Property Manager & Maintenance staff regularly.
Requirements EDUCATION AND/OR
Experience:
High school diploma or equivalent required with at least 3 years' of related experience, to include experience working with the older adult population or a bachelor's degree in a human services related field; gerontology or psychology preferred.
Working knowledge of supportive services and other resources for older adults and familiarity with common issues of aging and family caregiving is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must have a valid Driver's License and car - Motor vehicle check (MVR) with a satisfactory driving record required.
TRAVEL:
Day to day travel in the community may be required with some travel within the state for agency meetings, etc.
May include occasional overnight stays.

The check will bounce and the victim is left responsible.
