Our Care Managers use every aspect of their nursing training to teach Members how to advocate for their own Healthcare. Our Care Managers work with our Virtual Supervisory Team to provide clients and members with the best quality of care.
Care Managers assesse, educate, advocate, and refer on behalf of the members weekly.
Expectations: Care Managers will complete weekly visits in the Member's home. Here they perform assessments and provide education on how to best handle serious health care issues. Care Managers will create a Care Plan with the Member and follow it. Care Managers will be held to time sensitive situations and must document everything within 24 hours of any contact with the Members. Care Managers will have to communicate with the Virtual Team and Client Team regarding any concerns or expectations of the Members.
Requirements: Candidates will be an RN - MSW and have 1 year of Home Health Experience.
Candidates must be well organized, efficient, computer savvy, and able to work independently.
Candidates are responsible for having - Phone - Computer - Printer - Scanner (or fax)
Candidates must have Direct Deposit Accounts
Candidates must have Excellent Communication Skills - via Phone - via E-Mail - via Text - via Verbal - via Documentation
Candidates will need Professional Liability Insurance
Candidates will need to have a TB test within the last year.
Candidates will be drug screened
Candidates will have background checks monthly and prior to hire.
Professional Edge Nursing, INC is seeking Care Managers (RN - MSW). This is a per diem with great pay of $45 per visit and $30 for mileage after 100 miles driven in any given week.
Care Managers are in the home as advocates and coordinators to help Members access benefits and resources and as coaches to assist with disease management and self-care. Our goal is to prevent unnecessary hospitalizations and ER visits through three types of Programs Brief Assessment, 30-day Transition, and Long Term'.
A Brief Assessment is a one-time visit that has been requested to evaluate the member using the member survey, validate the areas of focus and provide recommendation.
The 30-day Transition program is for Members that are recently discharged from a hospital or rehab facility. A minimum of 3 visits are required are required to provide extra support.
The Long Term refers to members that have been referred to receive weekly face to face visits in the Long Term In-Home Program.
