Licensed Vocational Nurse - Hospice Inpatient Unit

Summary of Duties and
Responsibilities: Provides direct care to patients in our Compassionate Care Home (CCH) under the supervision of a Registered Nurse (RN) in accordance with Medical Director Orders and plan of care
Primary Department: Compassionate Care Home
Reporting Relationship: Registered Nurse Leader
Position Type: Non-Exempt, Part Time, Benefitted (24-32 hours per week)
Job Details and
Essential Functions:
o Under the direction and supervision of a Registered Nurse (RN), provides direct patient care including specialized duties related to end of life care
o Implements current nursing practice following the plan of care
o Provide accurate and timely documentation of patient services to reflect the plan of care in the Electronic Medical Record (EMR)
o Collects and provides patient and family/caregiver education and information pertinent to diagnosis and self plan of care as directed by the RN
o Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor.
o Uses and prepares equipment and supplies effectively, adhering to aseptic technique
o Provides appropriate pain/symptom management. Monitors patient's response to treatments/medications and reports changes to RN
o Contributes to patient and family counseling and education
o Participates in QAPI program and hospice sponsored in-service
o Participates in patient care meetings and HOF in-service and training
o Serves as back up to Hospice Aide
Knowledge and Skills
Required
o Commitment to principles, practices and procedures of nursing in a hospice environment
o Demonstrated competence in hospice care delivery techniques, with willingness to learn 'Most Current' hospice nursing competencies and procedures
o Dedication to 'Nursing Practices Act' record keeping requirements
o Attention to details of day-to-day operations of a Congregate Living health facility
o Routine nursing skills practiced in a clinical environment
o Intolerant of conditions that are symptomatic of neglect or abuse
o Providing accurate and timely records of critical events
o Cooperation with the Governing Body, COO, CEO, physicians, IDT members, and other CLHF coworkers
o Read, write, and communicate on a professional level
o Perform multiple tasks in a timely manner
Desired
o Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation
Education & Experience
Required
o Minimum one-year hospice experience or 2 years experience in a Skilled Nursing Facility
o Demonstrated competence in Hospice care delivery techniques
o Knowledge of terminal care and end-of-life issues
o Demonstrated communications and interpersonal skills, adaptability, organization, responsibility and leadership qualities
Desired
o Two or more years experience as an LVN for an inpatient Hospice Unit
o Knowledge of State and Medicare Hospice regulations
License / Certifications
Required
o Posses and maintain current California LVN license
o Posses and maintain current CPR certificate.
o Posses and maintain valid California driver's license, a reliable automobile and evidence of appropriate liability insuranceEstimated Salary: $20 to $28 per hour based on qualifications.
