We are currently seeking a LPN Case Manager for a client of ours in the healthcare
industry.
The LPN case manager will be responsible for coordinating care for Medicare patients with multiple chronic conditions. All care will be provided telephonically and it will be the responsibility of the LPN to relay pertinent information to all stakeholders. The case manager will be the primary link between the patient, their primary care provider, specialists, and the parent value-based services company. Excellent communication skills are a must. Strong candidates must be comfortable working independently and engaging in complex decision making.

Qualifications
Current active and unrestricted LPN/LVN licensure
Ability to apply for licensure in multiple states
Minimum of 3yrs clinical experience in an inpatient setting, hospital preferred
Previous case management experience
Previous experience with documenting in an electronic medical record (EMR)
Must possess the ability to make independent decisions when circumstances warrant such action
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to case management
Must be able to read, write, speak, and understand the English language.
Must be able to relate information concerning a patient's condition.
Must be able to navigate an electronic health record (EHR)
Knowledge of Microsoft Office including Outlook, Word, Excel, and PowerPoint
Superior organizational skills
Bilingual candidates a plus

Responsibilities
Utilizes the nursing process to assess, plan, implement and evaluate patient care
Performs focused interview to identify specific patient needs
Develop and implement individualized care plans for patients with multiple chronic conditions
Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses
Serve as a liaison between patients and their families and healthcare providers
Conduct thorough medication reconciliations
Mediate care transitions from an inpatient environment to home or home health
Coordinate home care resources for patients
Connect patients and their families to community resources
Relay information regarding patient's healthcare status to advanced practice providers as needed
Provide patient and provider education.
Identify related risk management quality concerns and report these scenarios to the appropriate resources.
