Nurse Practitioner- Hospice

We are seeking an experienced Nurse Practitioner who would be interested in providing direct patient care as part of our multidisciplinary team within a Medicare Certified Hospice Program.

Job Details and Essential Functions
o Collaborates with Hospice Medical Director/Supervising Physician to prescribe or recommend drugs or other forms of treatment and to identify appropriate differential diagnoses
o Monitors and maintains progress of patients and reports progress to Medical Director/Supervising Physician
o Records physical findings, and formulates plan and prognosis, based on patient's condition
o Discusses case with physician and other health professionals to prepare comprehensive patient care plan
o Provides on-call services and face to face patient visits
o Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT
o Performs ongoing assessment based on advanced nursing practice principles and specialty knowledge of patient population within scope of NP practice
o Collaborates with members of health care team in planning, implementing, monitoring and evaluating comprehensive healthcare for patients and families
o Documents care/services per guidelines and HOF compliance/billing practices
o Educates staff on new or advanced application of skills
o Precepts students and/or new staff
o Provides community education e.g., local colleges, physician offices
o Develops and maintains department specific competency program
o Assists with the evaluation of educational activities to determine their impact on the quality of care
Knowledge and Skills
Required
o Posses critical thinking skills, in-depth knowledge of clinical disease entities common to hospice and palliative care including pathophysiology, disease progression, pharmacological and non-pharmacological treatment modalities for palliative care, in-depth knowledge of the hospice philosophy, the Medicare hospice benefit
o Complete necessary documentation including history, physical assessment in electronic chart
o Promotes optimal health outcomes and development of goals incorporates interdisciplinary input
o Advanced nursing principles and practices applied per standardized procedures including furnishing medications as clinically indicated.
o Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation

o Participates in activities that promote the nursing profession, specialty area and advanced practice nursing
Education & Experience
Required
o A minimum of (2) years experience in hospice, home health or community health in the past five (5) years.
Desired
o Graduate of Accredited School offering an Advanced Degree in Nurse Practice (ARNP)
o Masters In Science Nursing or other related field
License / Certifications
Required
o California license in good standing both as a Registered Nurse and Nurse Practitioner
o Current BLS (Basic Life Support)
o Have national prescriptive drug privileges with a DEA number
o Possess and maintain valid California driver's license, a reliable automobile and evidence of appropriate liability insurance
Desired
o ANCC certification (ACNP; ARNP, FNP or GNP)Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
