Case Management Supervisor
We are seeking an experienced Case Management Supervisor for our 400
bed hospital client located in the Manteca area.
The Case Management Supervisor supervises Resource Utilization and Social Services team members to efficiently and effectively coordinate movement through the continuum of care for a defined population. The Resource Utilization/Case Management Supervisor serves as a clinical consultant to all Resource Utilization and Social Services team members, provides guidance and/or direct intervention in resolving challenging situations, and directly provides the full range of case management services for the highest risk, problem prone cases. The Resource Utilization/Case Management Supervisor has working knowledge of duties performed by team's Quality Management Coordinator (QMC), Quality Management Clerk and Social Workers and is capable of performing those duties in their absence. The Resource Utilization/Case Management Supervisor is responsible for maintaining current policies and procedures and ensuring compliance with applicable internal and external regulations. The incumbent is responsible for identifying operational barriers to effective provision of services, seeking resolution and for providing services in an efficient and cost-effective manner. The Resource Utilization/Case Management Supervisor additionally may carry a caseload and perform the function of the QMC.
Qualifications:
-Current California Registered Nurse License.
-Four (4) years recent acute hospital experience as a utilization management coordinator, case manager, or discharge planner OR Three (3) years recent acute hospital experience which includes experience in a role with leadership/supervisory responsibility including experience in utilization management, quality assessment, or discharge planning.
-Must possess excellent written and verbal interpersonal communication skills.
-Ability to communicate effectively with a wide variety of personalities and departments, including medical staff members.
-Must be able to travel in accordance with any requirements of your job
Estimated Salary: $20 to $28 per hour based on qualifications.
