Physical Therapists - Full Time (37.5 hours) and Per Visit
The Registered Physical Therapist provides rehabilitative and therapeutic care and services to patients in their homes, in accordance with physician referral. The Registered Physical Therapist assesses for and coordinates intervention of appropriate services with other community and Agency staff, supervises Physical Therapy Assistants, provides teaching to the nursing staff, home health aides and family members, and carries out special activities on assignment.
Responsibilities:
Evaluates patients and accurately identifies needs within 24 hours of receipt of referral or as indicated
Implements treatment plan, evaluating results of each treatment and adjusting plan accordingly
Demonstrates and teaches proper techniques to clinical staff, home health aides and family members, including range of motion and strengthening exercises, transfer activities, use of adaptive equipment, body positioning, and safe gait techniques
Provides direct patient care to wide range of patient population, demonstrating competent and up to date clinical skills
Effectively manages assigned caseload
Maintains effective interpersonal relationships and demonstrates effective communication skills
Requirements:
Bachelor's degree
Licensed in MA and NH
1-3 years experience
C.P.R. Certification
