Home Based Primary Care - Caregiver Support RN
The Caregiver Support Nurse is directly responsible to the Director of Home Based Primary Care (HBPC).
The Associate Chief Nursing Service (ACNS), Ambulatory Care and ultimately the Associate Director, Patient Care Services, serve as administrative resources and provide professional oversight to the Director of Home Based Primary Care.
These individuals collaboratively retain administrative, clinical, and disciplinary responsibility for the performance evaluation, time and attendance, assuring clinical practice complies with the scope of practice and clinical protocols and privileges for the Director of Home Based Primary Care.
The Caregiver Support Nurse provides leadership in the caregiver support program, serving as the nursing expert for the program.
The nurse will utilize clinical evidence-based services and interventions for the caregiver and Veteran; provide education and consultation for the caregiver, Veteran, and staff on caregiver issues; participate in community outreach, resource development, continuous quality improvement activities, and the evaluation of caregiver issues.
In addition, this nurse must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.
The Registered Nurse works collaboratively with the Social Work Caregiver Support Coordinator in delivering care to caregivers and Veterans in this program.
The Registered Nurse will function in support of caregivers, Veterans and family members throughout the medical center and in community settings.
This nurse is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies.
The RN is also administratively responsible and accountable for the clinical and program development and effectiveness of the facility Caregiver Support Program therefore, the nurse possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources.

Estimated Salary: $20 to $28 per hour based on qualifications.
