Care Manager, Long Term Support Services
We have an EXCELLENT opportunity for the right candidate to attain a leadership position with one the nation's most prominent non-profit managed care organizations! As the Care Manager of Long Term Support Services, you will conduct clinical assessments for members with long-term care needs through on-site and telephonic review. This individual will coordinate services and a care plan that meets the patient's psychosocial and clinical needs. The Care Manager will also provide member education, as well as maintain accurate documentation of ongoing assessments, interventions and outcomes.
WHAT WE LOOK FOR:
Master's Degree in Social Work, Psychology, Nursing or related field
Clear and active Licensed Clinical Social Worker (LCSW), Registered Nurse (RN) or other related clinical licensure in the state of New Hampshire
Extensive background in direct care services and working with populations receiving long-term support
Previous supervisory or leadership experience in a managed care setting preferred
Ability to travel up to 50% of the time a PLUS!
SALARY:
Up to $87,000 / year (DOE)
