Social Worker (Suicide Prevention Case Manager)
The incumbent provides veterans designated as high risk for suicide with high quality case management services, clinical treatment, advocacy, care coordination and linkage with appropriate VA services and/or community resources as needed or identified by the veteran and the family.
This is accomplished in collaboration with other interdisciplinary treatment providers both within the facility as well as interdisciplinary treatment providers in other facilities throughout the VA.
As a case manager, the incumbent demonstrates innovation, creativity, expertise and skill in an expeditious and efficient manner.
The incumbent functions as a team within the Suicide Prevention Program.
This position requires a current, valid driver's license, Incidental Driver Licensure as an independent social worker (LISW) or RNWork Schedule:
Monday - Friday, 1 st Shift 8:
00 am to 4:
30 pm Weekends and evenings may apply.
