Complex Care Registered Nurse
Judge healthcare is currently seeking Complex Care Registered Nurse for a full time permanent position with one of the premier health plans in New England!
The Complex Care RN will be primarily responsible for the care management of the most complex, highest risk members of the health plan.
This position will have the following duties:
Using assessments, motivational interviewing techniques and evidence based practices to engage with the members to develop an Individual Care Plan.
Assisting the member to access all available benefits and resources including family support and community resources.
Supporting and enhancing the member's capacity to self-manage.
Evaluating the effectiveness of the care management provided and updating the individual care plan accordingly.
Other duties as assigned
The candidate we seek will have the following
Qualifications:
BSN OR RN degree with sufficient work experience.
3 years clinical experience with complex care population.
3 years of home care or managed care experience.
2 years of case management experience.
Active MA RN license with no discipline.
Valid driver's license.
